Avoiding the sun is a risk factor of a similar magnitude as cigarette smoking in terms of life expectancy, new research now suggests. Scientists followed 30,000 women for 2 decades and found that those who avoided the sun were twice likely to die.

Sun Avoidance: A brief history

Guidelines which advise people to stay out of the sun have been around for a very long—and is harming the population, says Dr. Marc Sorenson, author of the books Embrace the Sun and Vitamin D3 and Solar Power for Optimal Health.

While our current culture largely views tanned skin as healthy & attractive, this was not always the case:

  • As far back as 3100 BC, the ancient Egyptians used extracts of rice, lupine & jasmine to protect their skin from the sun as a lighter skin tone was a symbol of high social status—indicating that the individual did not perform hard outdoor work. This phenomenon is comparable to the practice of feet binding in ancient China.
  • In 1100 BC, the Greeks were religiously avoiding the sun as they valued pale skin and used olive oil as sun protection if they ever had to leave the house.
  • The usage of chalk powder, white marl, crocodile dung and white lead to create a barrier between skin and the sun were also observed with the Romans as far back as 753 BC. The Roman recognition that lead was poisonous underscored their point of view: that pale skin was viewed as the most important feature of Roman beauty—a demarcation of the leisure class.
  • Extremely pale skin was made even more fashionable by Queen Elizabeth I of England during her reign in 1558–1603. After a bout with smallpox in 1562 left the young monarch with a lot of facial scars, the queen started painting her face with a concoction of white lead and vinegar (which gradually poisoned her over time). During this period, even freckles were considered undesirable as they signified a working class person who toiled in the sun.
  • In 18th-century Japan, the emerging popularity of the geisha made the toxic oshiroi (a white lead powder used for foundation, mixed with water before application, and then painted on with a wide, flat brush, over the neck, face & back) popular as well.
  • 1800’s—Skin cancer is discovered and the term “melanoma” is coined by French physician & inventor Rene Theophile Hyacinthe Laënnec. While still a medical student in 1804, Laennec became the first person to lecture on melanoma. This lecture would then published in 1805.
  • By 1932, Australian chemist Blake Milton (who since the 1920’s had been experimenting to produce what he called a “sunburn cream”) used the money he was finally able to raise from family and friends to launch his cream through Hamilton Laboratories (named after his transcript signature “H.A. Milton”). This was the world’s first-recorded commercially-available sunscreen. Meanwhile, French pharmacist Eugene Schueller (who had founded L'Oréal 23 years earlier in July of 1909) developed his own sunscreen formula in 1936r>Note: Both Hamilton sunscreen & L'Oréal sunscreen still exist as of the writing of this article.
  • The American Academy of Dermatology is founded in 1938he organization would popularize the narrative of “When you protect your skin from the sun, you reduce your risk of developing skin cancer and early skin aging.”
  • The U.S. Food and Drug Administration (FDA) standardizes the SPF calculation of sunscreen products in 1978.
  • As of 2020, the U.S. sunscreen manufacturing industry is a $406 million industry. Worldwide, revenues are projected to hit $13.7 billion by 2024.

The real

Fact #1: While rickets has long been considered a Third World disease, cases of rickets in the United States have risen fourfold in the last 15 years as sun avoidance & sunscreen have increased in popularity.

Fact #2: Similarly, the World Health Organization says that the number of adults with hypertension increased from 1975’s 594 million to 2015’s 1.13 billion.

Dr. Pisit Pitukcheewanont, Pediatric Endocrinologist & Director of Bone Disorders Program at the Children’s Hospital Los Angeles says that “appropriate sun exposure outweighs the risk of skin cancer, especially in children,” and recommends at least 20 minutes of noontime sun at least three times a week.

This is because Vitamin D is a requirement for calcium absorption and is non-negotiable for bone health.

The thing is—Vitamin D is only naturally produced by humans when the body reacts to the sunlight hitting the skin. This involves the synthesis of cholecalciferol in the lower layers of skin epidermis through a chemical reaction that is dependent on sun exposure (UVB radiation).

People with low levels of Vitamin D in their blood have significantly higher rates of virtually every disease & condition you can imagine: hypertension, diabetes, cancer, osteoporosis, heart attacks, stroke, depression, cognitive impairment, autoimmune conditions, etc.

And obtaining Vitamin D in sufficient amounts through food alone is also close to impossible as only a few food items like the flesh of oily fish, naturally contain significant quantities of Vitamin D. Plus, a person would have to eat fatty fish three or four times a week in order to get enough Vitamin D from this food source alone.

And if people are not able to get sufficient sunlight, then they will need to supplement with 1,000 IU of Vitamin D daily to stay healthy.

Dorothy "Dot" Bennett, Professor of Cell Biology at St. George's, University of London says that the ideal amount of sun exposure is “a little, rather than zero.”

This is because of the previous studies that have found that Vitamin D can increase survival rates for women with breast cancer—whereas low levels of Vitamin D have been linked to more aggressive forms of skin cancer. Vitamin D deficiencies also signal prostate cancer in men.

Vitamin D supplements versus sunshine for heart health

Low blood levels of Vitamin D are linked to an increased risk of heart attacks, stroke, heart failure and even death says Dr. Erin Michos, Associate Professor of Medicine at Johns Hopkins School of Medicine in Baltimore.

But Dr. Michos believes that the question of whether or not Vitamin D supplementation can actually strengthen the heart can only be answered more conclusively by larger clinical trials over the next 5-8 years. “Initially there was a lot of enthusiasm for Vitamin D treatment for cardiovascular disease, and this was based on observational data,” said Dr. Michos. “The question is, if you’re deficient and I give you Vitamin D back, can I actually prevent a heart attack?”

However, a study published in the Journal of Investigative Dermatology suggests that it is actual sunlight exposure that helps reduce blood pressure and thus cuts the risks for heart attacks & strokes.

“This is an unexpected finding, in that the skin has not been considered to be involved in blood pressure regulation,” said lead researcher Dr. Martin Feelisch, Cardiovascular Pharmacologist and Professor of Experimental Medicine and Integrative Biology at the University of Southampton.

Dr. Feelisch says that avoiding sunlight or using sunblock daily out of fear of skin cancer could be a new risk factor for heart disease.

Unlike Vitamin D supplements, Dr. Feelisch and his team found that actual sunlight causes the nitric oxide stored in the top layers of the skin to react, triggering blood vessels to widen as the oxide is moving through the bloodstream.

This, in turn, lowers blood pressure. “That's where it becomes interesting," he adds.

“Nitric oxide along with its breakdown products, known to be abundant in skin, is involved in the regulation of blood pressure. When exposed to sunlight, small amounts of nitric oxide are transferred from the skin to the circulation, lowering blood vessel tone; as blood pressure drops, so does the risk of heart attack and stroke,” Dr. Feelisch explains in further detail.

Sunshine for the heart

In Dr. Feelisch’s study, 24 healthy individuals were exposed to ultraviolet (UVA) light for two sessions of 20 minutes each.

Results found that the UVA exposure dilated blood vessels, significantly lowered the test subjects’ blood pressure numbers, and altered nitric oxide metabolite levels. Additional experiments indicated that stores of nitric oxide in the upper skin layers are involved in facilitating these effects.

The data was consistent with the seasonal variation of blood pressure & cardiovascular risks at temperate latitudes.

Dr. Feelisch adds: “These results are significant to the ongoing debate about potential health benefits of sunlight and the role of Vitamin D in this process. It may be an opportune time to reassess the risks and benefits of sunlight for human health and to take a fresh look at current public health advice. Avoiding excess sunlight exposure is critical to prevent skin cancer, but not being exposed to it at all, out of fear or as a result of a certain lifestyle, could increase the risk of cardiovascular disease. Perhaps with the exception of bone health, the effects of oral Vitamin D supplementation have been disappointing. We believe that nitric oxide from the skin is an important, so far overlooked contributor to cardiovascular health. In future studies we intend to test whether the effects hold true in a more chronic setting and identify new nutritional strategies targeted at maximizing the skin's ability to store nitric oxide and deliver it to the circulation more efficiently.”

In Summary

Sun avoidance is a practice that has been around since the beginning of civilization, but new research suggests that avoiding the sun entirely raises one’s risk for cardiovascular disease & other conditions. Vitamin D is necessary for bone health and low blood levels of Vitamin D are linked to an increased risk of heart attacks, stroke, heart failure and even death. However, larger clinical trials are necessary to see if taking Vitamin D supplements can in fact improve overall heart health. A little exposure to sunshine is better than having no exposure at all and some doctors recommend at least 20 minutes of noontime sun at least three times a week as a guideline. This is because obtaining Vitamin D in sufficient amounts through food alone is difficult as only a few food items contain significant quantities of Vitamin D.
